To begin, please complete the following:

1. Complete our Intake Form: View & Download Here
2. Ask your referrer to send any documents related to your referral, which may include: Pre-Sentence Investigation Report (PSI); DOC Conditions of Release; Psychosexual Evaluation; Prior Treatment Records; Prior Polygraph Reports; Criminal Complaint; Sentencing Order; County Subsidy Agreement; and/or Prison Release Documents (e.g. DOC custody summary)
3. Complete our Release of Information (ROI) form: View & Download Here
